As a selected participant in the Guiding an Improved Dementia Experience (GUIDE) model by the Centers for Medicare and Medicaid Services (CMS), Isaac Health is uniquely able to support caregivers whose loved ones are Medicare beneficiaries living with dementia. Our team is here to offer support, training, and guidance tailored to your needs.

Caregiver training
Isaac Health hosts group caregiver training sessions and dementia diagnosis information sessions for GUIDE Model members. These sessions will help you better understand dementia and what it may mean for your family. We also understand that every caregiving journey is unique, which is why we offer 1:1 coaching with specialized caregiver coaches.
Caregiver support groups
Our support groups provide a safe and nurturing space for caregivers to connect, share experiences, and find comfort in knowing that others understand what they’re going through. We can also connect you with local in-person support groups. These group sessions foster a sense of community and belonging and can promote emotional well-being.
Respite care
Everyone deserves to take a break. Respite care allows you to take a rest from caregiving, while a trained home care aide comes to your home to look after your loved one. As a member of Isaac Health’s GUIDE Model program, you will have up to 20 annual slots for respite care.

Frequently asked questions
To be eligible for GUIDE’s caregiver support, your loved one must be enrolled in GUIDE. For your loved one to be eligible for GUIDE, they must:
- Have Medicare Parts A and B as their primary insurance. They are not eligible if their primary insurance is Medicare Advantage or another Medicare health plan, including Special Needs Plans and the Program of All Inclusive Care for the Elderly.
- Not be enrolled in Medicare hospice benefit.
- Not be a long-term nursing home resident (defined as a nursing home stay of 4 months or longer and not covered under the Medicare skilled nursing facility benefit).
If your loved one is not eligible for GUIDE, you can still reach out to our Care Team to learn more about how Isaac Health’s other services can support you and your loved ones. We work with most major insurance plans for non-GUIDE services, which include assessment, treatment, and care management for dementia.
If your loved one is eligible for GUIDE, we will work with you and your loved one to enroll you in Isaac Health’s GUIDE Program. Once you are a member of our GUIDE program, you will be fully covered by Medicare for all services you receive through the program.
