For families in Montana affected by Alzheimer’s Disease and Related Dementias (ADRD), finding specialized care can be challenging — especially in rural areas. Montana is classified as a “dementia neurology desert,” meaning it is projected to have fewer than 10 neurologists available for every 10,000 people with dementia in 2025. Today, approximately 21,000 Montanans over the age of 65 live with ADRD, and an estimated 17,000 family caregivers provide 25 million hours of unpaid care each year.
To support these families, Isaac Health has partnered with Greater Good Health to provide much-needed dementia care and support services in Montana. Powered by the new CMS Guiding an Improved Dementia Experience (GUIDE) Model, this partnership expands access to essential services such as specialist dementia care and caregiver training, all fully covered by Medicare for eligible individuals.
What do patients and families get?
Through this partnership, Isaac Health and Greater Good Health are focused on improving the quality of life for those at all stages of dementia and their caregivers. Services provided by Isaac Health include:
- Specialized dementia care: Patients receive a thorough clinical evaluation and personalized care planning by specialist doctors.
- 24/7 support line: Families will have access to around-the-clock assistance and support.
- Personalized care coordination: Each patient and their family is paired with a Care Navigator, who helps them navigate the complexities of dementia care and get connected to community and online resources.
- Caregiver support and education: Caregivers will have access to 1:1 and group training sessions as well as respite care. These essential resources are designed to support their well-being while caring for their loved one.
