Isaac Health and Pearl Health Partner to Streamline Dementia Care through the CMS GUIDE Model

July 19, 2024 – Isaac Health, a leading national provider of brain health and dementia care, and Pearl Health, a leading technology company focused on physician enablement in value-based care, are excited to announce a strategic partnership to deliver the CMS Guiding an Improved Dementia Experience (GUIDE) Model to Traditional Medicare beneficiaries aligned with Pearl’s provider partners. This collaboration aims to enhance the quality of life for individuals living with dementia and alleviate the burden on their unpaid caregivers.

Today, approximately 6.9 million Americans live with Alzheimer’s disease or another form of dementia, according to the Alzheimer’s Association. There are an additional more than 11 million Americans who provide care – typically unpaid – for a family member or friend with dementia. Each caregiver averages nearly 31 hours of care per week, and 59% of family caregivers of people with dementia rate the emotional stress of caregiving as “high” or “very high” due to formidable challenges in coordinating care and finding necessary support.

The GUIDE Model is a nationwide program designed to improve the quality of life for people at all stages of dementia as well as their caregivers. As a GUIDE Model participant, Isaac Health provides much-needed care, support, and resources to patients living with dementia and their caregivers. Services offered include comprehensive assessments and care plans, care coordination, respite services, caregiver training, and 24/7 access to a support line. Enrolled beneficiaries and caregivers also have the assistance of personal Care Navigators, who help them access clinical and non-clinical services, such as support groups through community-based organizations.
