Isaac Health is honored to join the Alzheimer’s Association Dementia Care Navigation Roundtable (AADCNR), an initiative dedicated to improving the lives of individuals living with Alzheimer’s and related dementias (ADRD) through coordinated care models. The Roundtable convenes health systems, payers, clinicians, and researchers to establish consistent, equitable processes for dementia care navigation, ensuring comprehensive support for both individuals and their caregivers.
Through its participation, Isaac Health will contribute to workgroups addressing key areas such as technology and innovation and caregiver support. These efforts are designed to create actionable frameworks that minimize caregiver burden, reduce hospital readmissions, and support better overall outcomes for people living with dementia.
