Charlotte, NC and New York, NY, May 3rd, 2023 – Isaac Health, a leading brain health provider and health technology company, and Troy Medicare, a prominent regional Medicare Advantage plan, have joined forces in an innovative partnership to address the growing challenge of brain health conditions in the Medicare population.
Since December 2022, Isaac Health has been using its proprietary detection algorithms and data provided by MDPortals to screen Troy Medicare members at risk for Alzheimer’s disease and other brain health conditions. Isaac Health reached out to members flagged with an elevated risk and invited them for a brain health check-up, including further screening, medical assessment, and treatment as part of Isaac Health’s comprehensive brain health management program.
The program offered by Isaac Health includes a unique combination of tailored medical management by specially trained doctors and therapists, caregiver support, and proactive care management. The goal is to provide holistic and personalized care for patients with brain health conditions, addressing their physical, mental, and social well-being. It is the first program of its kind to address brain health at a population level.
The program has been off to a remarkable start. Out of the members reached out to, 38% engaged and enrolled in the program, with 87% following through for at least 3 months, indicating high patient and family member satisfaction and commitment. A total of 52 members have participated in the program so far.
