Subject Access Request Policy
As a patient of Isaac Health, you are entitled to request additional information regarding how we collect and use your personal data.
This process is frequently referred to as a Subject Access Request.
Once we receive your request, we’ll send you a confirmation. It can take anywhere from a couple of days to a month to compile your data, and you’ll receive an email when it’s ready.
We will do our best to respond promptly and in any event within one month of the following:
- Our receipt of your written request; or
- Our receipt of any further information we may ask you to provide to enable us to comply with your request, whichever happens to be later.
The information you supply in this form will only be used for the purposes of identifying the personal data you are requesting clarification, updating, and/or erasure of. You are not obliged to complete this form to make a request but doing so will make it easier for us to process your request quickly.

Data Subject Access Request. Please provide me with access to my personal data. If you select this option, we will provide you with a copy of the personal data we store on you as a healthcare provider, to the extent required by law.
Data Subject Erasure Request. Please delete my personal data. If you select this option, we will, in our capacity as a healthcare provider, delete your personal data under our control. We will also delete any personal data that Isaac Health processes about you at the time of your request to the extent required by law.
